Vaccine Requirements for School

The state of Indiana requires the following immunizations for children entering the 6th through 12th grades:
- MCV (to prevent Meningitis)
- Tdap (to prevent Tetanus, Diphtheria, and Pertussis)
Documentation of Chickenpox disease or two (2) Chickenpox shots.
The Adams County Health Department/Clinic has vaccine available for uninsured children to receive school required shots. Indiana State Department of Health is requiring that all parents with insurance contact their insurance provider before calling the health department as many companies are now covering shots. The Health Department is not able to vaccinate children with insurance coverage for shots. Those with Medicaid/Hoosier Healthwise should call their provider for services.

The requirements for Kindergarten and first grade entry are:
- 2 Chickenpox or PHYSICIAN documentation of disease
- 1 Polio shot after the age of 4 years
- DtaP after the age of 4 years
